Truthspren, symbolspren or symbolheads, are apparitions that are possibly a type of spren from Roshar. They are not directly visible; Elhokar sees them only in his peripheral vision, and Shallan captures them in her sketches of her surroundings. They are humanoid in shape in size, dressed in stiff, unnaturally straight robes. In place of faces, they have twisting, jagged symbols.

Symbolspren follow Shallan around during her struggle to make Jasnah's Soulcaster work. The symbolspren, or truthspren, as they may be called, ask a truth of the person attempting to Soulcast. If the person says something completely true, they are transported to Shadesmar, where they can Soulcast.
