This information has the ability to potentially ruin elements of the plot for the reader. Proceed with caution if you have not read this book.

The First Gem is a topaz with magical properties[1] that Hoid carries and from which he took his name for a while.[2] It was first named in the Second Oathbringer Letter when the author (presumed to be Autonomy or Patji) addressed Hoid as "Cephandrius, bearer of the First Gem." Hoid is currently trying to restore the gem from the dead, and was cautioned against doing so by Frost in The Second Letter in Words of Radiance.

Theories

It has been theorised that the First Gem was used in some form in the shattering of Adonalsium.[3]
