The footnote is wrong. There's 2 circumstances in which the effects are seen, the first time when Vin used it, she wasn't near death or being attacked or injured in anyway, but it still worked. She was just standing there being held by 2 inquisitors, seeing them as who they were and a younger lord ruler behind his older self. She wasn't near death when she used it until she killed the lord ruler. So the footnote I felt should be gone since there's nothing to back it and the book contradicts it.
